29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: In the storage buildings and boat launch areas, employees were exposed to struck by and crushing hazards while using a Hyster forklift, model S50XL, serial number A187V06693H in that additions and modifications which affected the capacity and safe operation of the forklift were made. The forks had been modified with holes to attached a ball to use as a hitch for moving empty boat trailers.
