Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to acts of workplace violence: (a) On or about August 14, 2013, where an employee was punched in the left side of the face and nose with a closed fist while the employee was passing medications at the home of HIT, Inc. clients located at 1511 South Washington Street in Bismarck, North Dakota. (b) On or about August 14, 2013, where an employee was struck in the middle of the back by a client, while the employee was assisting the client in getting ready for bed at the home of HIT, Inc. clients located at 1511 South Washington Street in Bismarck, North Dakota. (c) On or about August 22, 2013, where an employee was hit in the back by a client to whom the employee was passing medications at the home of HIT, Inc. clients located at 1511 South Washington Street in Bismarck, North Dakota. (d) On or about August 22, 2013, where an employee was mopping the floor and was punched in the face with a closed fist by a client at the home of HIT, Inc. clients located at 1511 South Washington Street in Bismarck, North Dakota. (e) On or about August 27, 2013, where an employee was slapped in the face by a client to whom the employee was passing medications at the home of HIT, Inc. clients located at 1511 South Washington Street in Bismarck, North Dakota.
