29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): (a) Facility, 201 North Lehigh Avenue, Sayre, PA: Employees operated powered industrial trucks (forklifts) daily and did not receive training and an evaluation of their ability to safely operate the equipment. Employees were exposed to struck-by hazards associated with forklift operations, on or about August 30, 2013. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Printing Press Area, 201 North Lehigh Avenue, Sayre, PA: An employee adjusted the ink feed throw-offs on the top part of the DGM 430 4-High Tower Unit printing press without the 10 and 13-side outside roller guards in place to limit employee exposure to ingoing nip points and rotating parts while the press was in operation. The employee was exposed to caught-in and caught-between hazards, as observed on or about September 12, 2013. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.
