29 CFR 1926.20(b)(2): Programs did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. On or about 29 August 2013, and at times prior thereto, the employer did not ensure that frequent and regular inspections of the loading and securing for transport of LoDril auger units were being made at various job sites, exposing employees involved with that operation to the hazard of being struck by flying objects.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury. On or about 29 August 2013, and at times prior thereto, the employer did not develop and instruct each employee on a procedure to load and secure a LoDril Auger, Model DH60 drill rig to a low boy trailer for transport, to include the recognition and avoidance of unsafe conditions applicable to the work environment such as, but not limited to stowing the winch hook, exposing employees to the hazard of being struck by a wire rope service line and hook.
