On or about August 21, 2013, northeast corner of the building, the 3 buck high scaffold system on the top and second levels were not fully planked, exposing employees to a 20 feet fall hazard. On or about August 21, 2013, northeast side of the building, two employees cutting and installing tile on the top level of the 3 buck high scaffold system were not protected from falling 20 feet high by the use of a guardrail system, safety net system or a personal fall arrest system.
