On or about 8/28/13, shop, a Primeline hydraulic press brake lacked point of operation guarding in that the two hand controls were defeated by a lever there-by exposing employees to amputation hazards. b. On or about 8/28/13, shop, a Chicago hydraulic press brake lacked point of operation guarding and lacked shrouds on the left and right foot pedals there-by exposing employees to amputation hazards. c. On or about 8/28/13, shop, a Scotchman 5014 ironworker lacked point of operation guarding for the 3 station punch presses there-by exposing employees to amputation hazards. d. On or about 8/28/13, shop, a Guifil hydraulic shear lacked point of operation guarding at sides and back of shear there-by exposing employees to amputation hazards.
