29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On August 28, 2012, at a jobsite located at 923 W. Battlefield Rd, Kansas City, MO., Three employees engaged in shealthing work on a steep pitched roof were exposed to serious fall hazards, in that no Fall Protection Equipment or Fall Protection Systems were in use. Employees were exposed to serious injuries from fall hazards from a height of greater than 22 feet. Abatement certification for this violation is not required.

Battlefield Rd, Kansas City, MO., Three employees engaged in shealthing operations work were using portable ladders that did not extend 3ft above the landing surfaces. Employees were exposed to serious injuries from fall hazards ranging from approx. 11 feet to approx 17 feet above lower levels. Abatement certification for this violation is not required.
