OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees     On or about August 30, 2013 and at times prior thereto, the employer did not ensure that frequent inspections of hoist chains, and hooks, with signed and dated reports were performed to detect excessive wear, twist, or deforamtion.   The chains and hooks were used to move premanufactured steel pieces and manufactured steel parts.  The steel pieces and manufactured parts ranged in weight from one hundred (100) to eight hundred (800) pounds. Employees were exposed to being struck by or crushed by falling loads in the event of hoist chain or hook failure on the following mobile floor cranes:      a)  Crane Master 1 Ton located in the 460 Building.   b)  Dayton Crane, 1500 pound, located in the Job Shop.     To abate this violation the employer must ensure that frequent inspections of hoist chains and hooks, with signed and dated reports are performed to detect excessive wear, twist, deformation or damage of hooks.       Pursuant to 20 CFR 1903.19(d), the employer must provide documentation that this violation has been abated within fifteen (15) working days from the date of this citation.

The hoists were used to move premanufactured steel pieces and manufactured parts   The premanufactured steel pieces and manufactured parts ranged in weight from one hundred (100) to eight hundred (800) pounds.  Employees were exposed to being struck or crushed by a falling load in the event of a failure of the hoist on the following mobile floor cranes:   a) Crane Master 1 Ton located in the 460 Building,   b) Dayton Crane, 1500 pound, located in the Job Shop.     To abate this violation the employer must ensure that periodic inspections of the hoist are performed.     Pursuant to 20 CFR 1903.19(d) the employer must provide documentation that this violation has been abated within fifteen (15) working days from the date of this citation.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     On or about August 30, 2013, and at times prior thereto, the employer did not ensure machine guarding was provided on the following machines:   a) CNC Lathe, located in the 460 Building, the doors were not interlocking and the lathe will continue operating when the doors are opened.   b) Hemsaw Vertical Saw, the unused portion of the blade was not guarded.   Employees were exposed to the hazard of amputations.
