29 CFR 1910.36(b)(1): At least two exits routes were not available to permit prompt evacuation of employees during an emergency: Drilling rig derrick: On or about August 29, 2013, employees were providing well casing installation services during the drilling of an oil/gas well. The employee working from an elevated position on the rig derrick had only one means of egress that may be blocked by fire. This exposed the employee to the hazard of fire/explosion.

29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects: East end of the drilling rig: On or about August 29, 2013, the casing crew was preparing to insert intermediate casing into an oil/gas well. Three of the crew employees were not wearing their hard hats during the rig up exposing them to the hazard of being struck by falling objects.
