29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: (a) Press Room: Employees were exposed to electrical hazards as equipment, such as, but not limited to, the Mitsubishi Four Color Press, and the Mueller stitcher, do not have machine specific procedures for lockout tagout explaining where to lockout and tagout each piece of equipment that is hardwired in, has more than one power source, or is not locked out at the machine. Abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Bindery Pressroom: Employees were exposed to rotating parts as the Saddlestitch Binder had the plexiglass guard broken on the east end of the machine. 29 CFR 1910.305(g)(1)(iv): Flexible cords and/ or cables were used for purposes prohibited by paragraphs (A) through (F) of 29 CFR 1910.305(g)(1)(iv): (a) Prepress: Employees were exposed to electrical hazards as the electrical cord being used as a cord for the metal pendant box with prepunched knockouts was subject to abrasion as the cord was run through the ceiling. Flexible cord can not be run through holes in ceilings. Abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
