On or about August 30, 2013, at the above addressed jobsite, three employees were operating a rough-terrain/self-propelled crane, which was not inspected daily, and/or monthly, and for which was known by the operator to have an inoperative "operational aid" (anti two-blocking device), and for which a "temporary alternative measure" had not been implemented, which resulted in the auxiliary hoist rope failure.

On or about August 30, 2013, at the above addressed jobsite, three employees were operating a rough-terrain/self-propelled crane, which was not inspected annually, and for which was known by the operator to have an inoperative load indicator, boom angle indicator, or other inoperative indicator (anti two-blocking device); subpart(d)(4)(vi), and for which a "temporary alternative measure" had not been implemented, which resulted in the auxiliary hoist rope failure.
