Live parts of electric equipment were not guarded against accidental contact by cabinets or other forms of enclosures, nor by any of the means listed in subparagraphs (A) through (D) of this paragraph. LOCATION; 391 Broadway, New York, NY electric service panels at each floor. On or about September 4, 2013; a). Employees were working on the third floor performing clean up and debris removal in close proximity to the electrical service panel on the thrid floor. The elctrical service panel was energized and live as extension chords and temorary lighting were supplied from this panel.

29 CFR 1926.405(a)(2)(ii)(E): All lamps for general illumination shall be protected from accidental contact or breakage. Metal-case sockets shall be grounded. Location: 391 Broadway, New York, NY 10002, Throughout project. On or about September 4, 2013; a). Employees were working througout the project performing clean up and mason duties in the form of sweeping and setting concrete block in close proximity to temporary lighting fixtures that were not prtected from accidental breakage or contact.

Location 391 Braodway, New York, NY 10002. On or about September 4, 2013; a). Employees were exposed to falls in excess of 8 feet to the floor below while working in close proximity to floor edges around elevator shaft and stairwells. Employees were not protected from fall hazards by any means of Personal Fall Arrest system, or guards or handrails around said openings.

Employees were observed using the stairs to traverse throughout the project. The stair systems were not provided with handrails to prevent worker injuries from slip, trips or falls.
