29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose: This violation was discovered on August 29, 2013 at the Punahou School; where a 277 Volt electrical panel box had hand written labels that did not correspond to the individual circuit breaker switches to indicate its purpose.

29 CFR 1910.333(b)(2)(iii)(A): A lock and a tag was not placed on each disconnecting means used to deenergize circuits and equipment on which work is to be performed. This violation occurred on August 28, 2013 at the Punahou School; where a circuit in the 277-Volt electrical panel box was not locked out and tagged out while an employee stripped electrical wires on a 4-way switch system that was accidentally energized.
