29 CFR 1926.1053(b)(6): Ladders were not used on level surfaces, nor secured to prevent accidental displacement: a) Job Site, School Theatre, 1st floor: A 32 foot aluminum extension ladder used by employee to paint the wall was set up on an uneven surface and was not secured, on or about 8/17/13. Employee was exposed to falling approximately 18' to the concrete floor.
