29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system:   a) Job site, 1301 Biltmore Drive, Pittsburgh, PA: On or about August 29, 2013, during roofing activities on a roof with an approximate 8/12 pitch, the employer did not ensure that appropriate fall protection systems/devices were in use where employees were exposed to fall hazards of up to approximately 30 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards that enabled each employee to recognize the hazards of falling and trained each employee in the procedures to be followed in order to minimize these hazards:   a) Job site, 1301 Biltmore Drive, Pittsburgh, PA: On or about August 29, 2013, prior to performing roofing activities on a roof with an approximate 8/12 pitch, the employer did not ensure that appropriate fall protection training was provided for employees in that they were relying on roof jacks for fall protection and had no other fall protection systems/devices in use where exposed to fall hazards of up to approximately 30 feet.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:  a) Job site, 1301 Biltmore Drive, Pittsburgh, PA: On or about August 29, 2013, during roofing activities, the employer did not ensure that ladders used at the job site extended at least 3 feet above the landing surface or that they were secured to prevent movement and a grab bar was installed, exposing employees to fall hazards of up to approximately 10 feet.

Employees used ladders at the worksite that were not extended at least 3 feet above the landing surface, nor were they secured and a grab bar installed, exposing them to fall hazards of up to approximately 10 feet.
