29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measures: JOB SITE: 2664 S. 59th Street, St. Louis, MO 63139, at this work site, workers were observed on top of a 6:12 pitch roof, approximately 12 feet 11 inches eave to ground level, actively tearing off & removing old roof shingles, and were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measures. The workers were exposed to a fall hazard.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, when such an extension is not possible because of the ladder's length, the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, was provided to assist employees in mounting and dismounting the ladder: JOB SITE: 2664 S. 59th Street, Saint Louis, MO 63139, at this work site, workers were allowed to use a Werner aluminum extension ladder, MODEL: D1224-2, which was pulled apart to create to two (2) separate ladders, to gain access onto the roof, approximately 9 feet 7 inches eave to ground level. The ladder side rails did not extend at least 3 feet to the upper landing surface and the ladder was not secured to prevent from deflecting. The workers were exposed to fall hazard.
