29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l)  (Construction Reference 1926.602):   Workers were observed engaged in residential construction activities re-roofing an existing multi-unit condominium complex. Workers were exposed to potential struck-by and secondary fall protection hazards in that the operator had not completed a certification training program.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    Workers were observed engaged in residential construction activities re-roofing an existing multi-unit condominium complex. Workers were exposed to potential 19.5 foot fall hazards in that a proper fall protection system was not utilized.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:     Workers were observed engaged in residential construction activities re-roofing an existing multi-unit condominium complex. Workers were exposed to potential 19.5 foot fall hazards in that the workers were not properly trained in fall protection criteria.
