29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: (a) Facility Wide - Employees were not using the necessary personal protective equipment such as heat resistant sleeves to prevent burn injuries when manually removing screws, with surface temperatures of 90 degrees or higher, from injection molding machines to prevent burn injuries. On August 1, 2013, an employee removing a screw, with a temperature of 90 degrees or higher, from an injection molding machine suffered 2nd degree burns when the screw being removed slipped off the roundsling and brushed against the employees arm.

29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the customer or user without the manufacturer's prior written approval: (a)Facility Wide - A Toyota forklift (serial#11855) was in operation with a hole near the tip of the blade which was used to attach an eye bolt for material handling using slings without manufacturer's prior written approval to operate the powered industrial truck in that condition.
