29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary:    On or about 8/27/2013, at the above addressed jobsite, an employee was engaged in leveling measurements and installation of exterior wall tile from a scaffold at approximately 18 feet above ground level with the platform not fully planked, exposing him to a fall hazard.

On or about 8/27/2013, at the above addressed jobsite, an employee was accessing a residential roof by means of an 8 feet A-frame aluminum step ladder used in a closed position resting to the wall on top of an 18 feet scaffold platform, exposing the employee to a fall hazard.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:    On or about 8/27/2013, at the above addressed jobsite, an employee was engaged in leveling measurements and installation of exterior wall tile from a scaffold at approximately 18 feet above ground level with no guardrails, exposing the employee to a fall hazard.      C & E Imports, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1)(vii), which was contained in OSHA inspection number 835963 citation number 1, item number 1, and was affirmed as a final order on 2/22/2013 with respect to a workplace located at 2724 Sea Island Dr. Fort Lauderdale, FL 33301.
