29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:  Glue spray booth: On or about October 22, 2013, employee(s) performing spraying procedures with Wilsonart® 800 Adhesive (contains: Acetone, Cyclohexane, n-Hexane, n-Pentane, Toluene) in a glue booth were exposed to a hazard of undue physiological burden from the use of a tight-fitting, half mask respirator without being provided the precautionary respirator use information in Appendix D to 29 CFR 1910.134.

29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:  (a) Glue spray booth:  On or about October 22, 2013, employees were exposed to a hazard of undue physiological burden from the use of half-mask, tight-fitting respirators without first receiving a medical evaluation to determine that the employees were medically able to use that respirator.  (b) Glue spray booth: On or about September 5, 2013, employees were exposed to a health hazard from the shared use of one AOSAFETY half-mask, tight-fitting respirator that was stored in the glue booth area which the employer did not ensure was cleaned, stored and maintained so as to not present a health risk to employees.
