29 CFR 1910.147(c)(4)(ii): The procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C), and (c)(4)(ii)(D):   a)  Machine Shop, east wall, Acurpress hydraulic press brake, Model No. 7606, Serial No. 10139:   On or about September 4, 2013, employees were exposed to the release of hazardous energy, in that the written energy control procedures did not specifically identify all energy sources, such as gravitational, hydraulic and electrical energy, for employees engaged in the performance of servicing and/or maintenance activities.   b)  Machine Shop, east wall, Acurpress hydraulic press brake, Model No. 7608, Serial No. 11192:   On or about September 4, 2013, employees were exposed to the release of hazardous energy, in that the written energy control procedures did not specifically identlfy all energy sources, such as gravitational, hydraulic and electrical energy, for employees engaged in the performance of servicing and/or maintenance activities.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:   a) Machine Shop, east wall, Acurpress hydraulic press brake, Model No. 7606, Serial No. 10139:  On or about September 4, 2013, employees were exposed to hazardous energy including but not limited to electrical at 460 volts, as well as gravitational energy, while conducting maintenance activities.   b) Machine Shop, east wall, Acurpress hydraulic press brake, Model No. 7608, Serial No. 11192:  On or about September 4, 2013, employees were exposed to hazardous energy including but not limited to electrical at 460 volts, as well as gravitational energy, while conducting maintenance activities.

29 CFR 1910.212(a)(3)(ii):  The point of operation of machines whose operation exposes an employee to injury, was not guarded to prevent the operator from having any part of his body in the danger zone during the operating cycle:   a)  Machine shop, east wall, Accurpress hydraulic 60-ton press brake, Model No. 7608, Serial No. 11192:  On or about September 4, 2013, employee(s) were exposed to an amputation hazard while operating the press brake, as the employees' hands are approximately 2 inches from the unguarded point of operation.  b)  Machine Shop, east wall, Accurpress hydraulic 60-ton press brake, Model No. 7606, Serial No. 10139:  On or about September 4, 2013, employees were exposed to a caught-in hazard at the unguarded rear area of the 6-foot  press brake, while they stored and retrieved tools.  c)  Machine Shop, east wall, Accurpress hydraulic 60-ton press brake, Model No. 7608, Serial No. 11192:  On or about September 4, 2013, employees were exposed to a caught-in hazard at the unguarded rear area of the 8-foot press brake, while they stored and retrieved tools.
