29 CFR 1926.452(w)(1): Scaffolds were not plumb, level, and squared: (a) Location - 353 Cambridge Street, Winchester, MA: On or about 9/4/2013, a fully elevated Skyjack SJIII Model 3219 scissor lift was in use by an employee during the manual demolition of soffit while parked on a slope in excess of its rated capacity.

29 CFR 1926.452(w)(6)(i): Employee(s) were allowed to ride on scaffolds when the surface on which the scaffold was moved was not within 3 degrees of level, and/or not free of pits, holes, and obstructions: (a) Location - 353 Cambridge Street, Winchester, MA: On or about 9/4/2013, employees used a handicapped access ramp with a slope in excess of 3 degrees to access a side walk from which they were operating a Skyjack SJIII Model 3219 during the manual demolition of a soffit.
