29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant, chemical state and physical form: On or about September 4, 2013 and times prior thereto the employer did not evaluate the respiratory hazards associated with the bulk use of dry ice iinside a building. The sublimation of the dry ice in the deboning area exposed employees to the hazards of elevated levels of carbon dioxide.
