29 CFR 1926.50(e): Proper equipment for prompt transportation of injured person(s) to a physician or hospital or a communication system for contacting necessary ambulance service, was not provided On or about September 4, 2013, and times prior thereto, at a remote construction site, provisions were not made for: A. The prompt transportation of employees who were exposed to serious injuries as a result of falls from heights while working from the elevated basket of a Genie aerial lift, model S-65, self-propelled telescopic boom, to a nearby medical facility. B. A communication system which would permit the immediate notification of the need for emergency services to the nearest emergency services provider.

The training did not include the following topic: At this job site, employees performing bridge building activities from a Genie, model S-65, aerial lift at heights over 35 feet were not instructed in the safe operation of of the aerial lift and recognition of hazards associated with the work.
