29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  a) On or about September 6, 2013 and at times prior thereto at the jobsite; employees were working from a ladder jack scaffold approximately 20' in height with no fall arrest system in place.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a)  On or about September 6 and at times prior thereto at the jobsite; employees were working on a 12 on 12 pitched roof approximately 24 feet from ground to eave using only slideguards for protection.
