29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2): a) On or about September 5, 2013, the employer did not verify through written certification that a personal protective equipment hazard assessment had been conducted for employees of the spin blast machine #1 area. Employees of the area were exposed to eye hazards and overhead hazards necessitating the use of personal protective equipment. b) The employer did not verify through written certification that a personal protective equipment hazard assessment had been conducted for all required personal protective equipment in all areas of the plant. ABATEMENT DOCUMENTATION AND ABATEMENT CERTIFICATION REQUIRED.

29 CFR 1910.212(a)(2): Machine guard(s) on machine(s) were so installed that they offered accident hazards in themselves: a) An overhead plate on the outfeed left hand side of spin blast machine #1 was tack welded and not safely secured to the machine. On August 1, 2013 the plate on the spin blast machine #1 in the warehouse fell and hit an employee on the head. NO ABATEMENT DOCUMENTATION AND ABATEMENT CERTIFICATION REQUIRED.
