29 CFR 1926.451(g)(1)(i):   Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system.      a)  Worksite, 210 Ocean Ave. Amityville, NY - Employee was working on a ladder jack scaffold approximately 20ft above the ground without being protected by a personal fall arrest system; on or about 09/6/13.       Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge:    a) Worksite, 210 Ocean Ave., Amityville NY - Employees were gaining access to the interior of the home while climbing steps that had unprotected sides.  The employees were exposed to falling up to 9ft to the ground below; on or about 9/6/13.      Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
