29 CFR 1910.134(c)(1): In any workplace where respirators are necessary to protect the health of the employee or whenever respirators are required by the employer, the employer did not establish and implement a written respiratory protection program with worksite-specific procedures:    a). On or about 9/6/13, on site, in the LKMD room, for the employee required to don a half mask respirator when the employee is mixing thinner 521 and IA33 into the reservoir of the machine. The employer did not provide a respiratory protection program that included: a written program; fit testing procedures; medical evaluations of respirator users; procedures for proper use of respirators; procedures and schedules for cleaning, disinfecting, storing, inspecting, repairing and discarding; training on respiratory hazards and proper use of respirators; and procedures for regularly evaluating the effectiveness of the program.  b). On or about 9/6/13, on site, in the LKMD room, for the employee required to don half mask respirator when using IA20 to touch up the circuit boards. The employer did not provide a respiratory protection program that included the elements listed in paragraph a) above.

On or about 09/06/2013, at the shop/metal fab area, for employees who operate the Amada Model #FBD1025NT power press on a daily basis. Lack of guarding exposed employees to an amputation hazard when feeding metal pieces to the machine without point of operation guarding.
