On or about 09/09/2013, the employer did not ensure that tripping hazards were removed or identified to alert employees of the hazard in the shipping department. At the overhead door in the NE corner the floor between the loading dock and shipping dept had an offset of approx 1.25 inches, that was not marked, creating a tripping hazard for employees entering the shipping dept from the dock.
