29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. On or about 09/09/2013, the employer did not ensure that tripping hazards were removed or identified to alert employees of the hazard in the shipping department. At the overhead door in the NE corner the floor between the loading dock and shipping dept had an offset of approx 1.25 inches, that was not marked, creating a tripping hazard for employees entering the shipping dept from the dock.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about 09/09/2013, the employer did not ensure that various machines, listed below, located in the machine ahop area of the facility, were not equipped with guards. INSTANCES: a) Milling machine, Mfgr: TREE, Property Tag #135: A point of operation guard was not provided to prevent exposure to rotating parts and flying chips. b) Drill Press, Mfgr: Rockwell, No other identifying information:A point of operation guard was not provided to prevent exposure to rotating parts and flying chips. c) Horizontal Band Saw, Mfgr: Kysr/Johnson, Model #HA250: A blade guard was not installed over the moving blade on the left side of the point of operation.
