On or about September 5, 2013, an employee was operating an excavator approximately 7 feet from high voltage electrical power lines while digging an excavation, exposing employees to an electrical hazard.

29 CFR 1926.651(i)(3): Sidewalks, pavements, or appurtenant structures had been undermined and a support system or another method was not provided to protect employees from the possible collapse of such structures:  a.  On or about September 5, 2013, an employee operating an excavator undermined the asphalt pavement while conducting trench and excavation operations, exposing employees to cave-in hazards.

29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary:  a.  On or about September 5, 2013, an employee operating an excavator placed the spoils directly on the edge of the excavation, exposing employees to the hazard of large chunks of soil and other debris rolling back into the excavation.

On or about September 5, 2013, in the 8 feet 5-inch deep excavation, employees were not protected from cave-ins by the use of a trench box, shoring, or sloping, exposing employees to cave-in hazards.

On or about September 5, 2013, employees were using a 20-foot defective fiberglass extension ladder for egress and access while conducting trenching and excavation operations.
