Scaffolds shall not be altered or moved horizontally while they are in use or occupied. a) South east corner of west extension of exterior wall, 9th floor - On or about 8/22/13, the rigging line of the boatswain's chair scaffold was not maintained in a safe condition in that it was not protected from contact with any surface that could abrade, sever, weaken or damage it. An employee was conducting window washing activities from the 9th floor of the 26 story building.

29 CFR 1910.28(j)(4): The worker was not protected by a safety life belt attached to a lifeline securely attached to substantial members of the structure (not scaffold) or to securely rigged lines, which would safely suspend the worker in case of a fall: a) South east corner of west extension of exterior wall, 9th floor - On or about 8/22/13, the safety lines being used for personal fall arrest systems were not independently rigged from the anchorage of the boatswain's chairs. Employees were conducting window washing activities from the 9th floor of the 26 story building.
