29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.  (a) Front of Building - On or about September 5, 2013 two employees was exposed to a fall hazard while climbing on a metal frame scaffold.  There was no ladder attached to scaffold for access. Employees were exposed to falling 7 feet to ground.

29 CFR 1910.1200(g)(8): The employer shall maintain in the workplace copies of the required material safety data sheets for each hazardous chemical, and shall ensure that they are readily accessible during each work shift to employees when they are in their work area(s). (Electronic access, microfiche, and other alternatives to maintaining paper copies of the material safety data sheets are permitted as long as no barriers to immediate employee access in each workplace are created by such options.)    (a)  Construction Site - On or about September 5, 2013 the employer did not have a material safety data sheet (MSDS) for corrosive liquid, Vana Trol.
