This hazard is with respect to a workplace located at 4600 Rocky River Drive, Cleveland, OH.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    On or aboout Sepetember 5, 2013, employees engaged in the mixing and use of concrete or mortar were not utilizing eye protection to prevent incidental contact. Employees were mixing mortar on the northern side of the construction site located at 4600 Rocky River Drive, Cleveland, OH.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.     On or about September 5, 2013, employees gained access to the frame type scaffolding on site at 4600 Rocky River Drive, Cleveland, OH by utilizing the crossbracing. Ladders were available on site but were not utilized.
