At the site - employees required to wear respiratory protection during granite cutting were not fit tested. Employees wore a 3M N-95 filtering face piece device and a half face elastomeric respirator. Violation observed on or about 09/10/13.

29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or  were not packed or stored to prevent deformation of the facepiece and exhalation valve:  a. At the site - a half face elastomeric respirator was not protected from silica dust, in that, the respirator was stored on top of the work bench near the dry saw. The respirator was left open on the bench and had visible dust on the outside and inside of the respirator. Violation observed on or about 09/10/13.

Cutting side - employees were exposed to silica dust, in that, the floors had accumulated granite dust and were not kept clean. Violation observed on or about 09/10/13.    b. Cutting side and fabrication side - employees were exposed to tripping hazards, in that, the floors were not kept clean. There were pallets on the floor and sticks along the passage ways. Violation observed on or about 09/10/13.
