29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows:       (a)    Right-side of House - On or about September 6, 2013 employees were exposed to a fall hazard while working on a scaffold that was not fully decked or planked. This scaffold had a 24 inch gap in the scaffold platform. The scaffold platform was 16.6 feet above ground.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers.   Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part.     (a)  Right-side of House - On or about September 6, 2013 employees were exposed to a fall hazard while working on a welded metal frame scaffold 16.6 feet above ground. Employees were not protected from falling to a lower level by a guardrail system or personal fall arrest system.

29 CFR 1926.452(c)(2): Frames and panels shall be braced by cross, horizontal, or diagonal braces, or combination thereof, which secure vertical members together laterally. The cross braces shall be of such length as will automatically square and align vertical members so that the erected scaffold is always plumb, level, and square. All brace connections shall be secured.     (a)  Right-side of House - On or about September 6, 2013 employees were exposed to a fall hazard while working on a scaffold that was missing 3 sets of cross-braces on the back-side.

Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.     (a)  Front-side of Roof - On or about September 6, 2013 four employees were exposed to a fall hazard while working on a steep roof 25 feet above ground. The roof had a pitch of 6/12. Employees were not protected from falling to a lower level by a guardrail system or personal fall arrest system or safety net system.
