29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about 9/10/2013, at the above addressed jobsite, the employer did not provide training to employees in the use of fall protection and recognition of fall hazards while engaged in commercial roofing work, at 13 feet 3 inches above ground level, exposing the to a fall hazard.    NOTE: CERTIFICATION OF ABATEMENT MUST BE SUBMITTED FOR THIS ITEM.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about 9/10/2013, at the above addressed jobsite, employees were using a 32 feet extension ladder that did not extend at least 3 feet above the upper level or landing of a commercial roof with a slope of 3:12 at 13 feet 3 inches above ground level, exposing the employees to a fall hazard.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems or a combination of warning line system and safety monitoring system:    On or about 9/10/2013, at the above addressed jobsite, employees were engaged in roof tile installation from a commercial roof at 13 feet 3 inches above ground level without a means of fall protection, exposing them to a fall hazard.    Kessel, Inc. dba Atlas Roofing was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 313207409 citation number 2, item number 1 and was affirmed as a final order on 12/21/2010, with respect to a workplace located at 200 Kendall St. Springfield, MA 01103.
