(a) Construction Site - On or about September 9, 2013 employees were exposed to thrown from forklift. Forklift operator was not wearing seatbelt on a JLG Lull 1044C-54 (Series II Forklift serial no. 15787) forklift.

Employees were not protected from falling to lower level by a guardrail system or personal fall arrest system or safety net system.
