29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling objects:   On or about 09/10/13 at 1600 Marietta Hwy, in Canton, GA: Employees working on the ground without a hard hat were exposed to struck-by hazards where shingles were being thrown from the roof to the lower level.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system.  On or about 09/10/13 at 1600 Marietta Hwy,  in Canton, GA. The employer failed to provide fall protection for employees that were exposed to fall hazards of up to 19 feet while working on the roof installing roofing shingles.
