29 CFR 1910.213(i)(1): Nonworking portion(s) of the blade of bandsaw(s) were not enclosed or guarded: On or about September 10, 2013 and times prior thereto, an employee of D.I.M. Way, LLC was observed using a horizontal band saw without a guard, exposing the employee to amputation hazards in the event either his fingers or hand were to come into contact with the blade.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws. On or about September 10, 2013 the horizontal band saw at D.I.M. Way, LLC was observed to be operated by employees with the bushing not attached to the metal motor box to provide strain relief and prevent the pull of the cable.
