29 CFR 1910.303(b)(2): Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing or labeling: 1) On September 11, 2013, and at prior times, an outlet box designed to be installed on a wall was used as an extension box for a 220 volt welder. The outlet box was laying on the floor and had a damaged knock out.

29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored and maintained so as not to create a health hazard to the user: 1) On September 11, 2013, the employer provided body tech employees with tight fitting half mask respirators for use on a voluntary basis. The employer had not provided a medical evaluation to the employees.
