29 CFR 1915.73(d): When employees were exposed to unguarded edges of decks, platforms, flats, or similar flat surfaces, more than 5 feet above a solid surface, the edges were not guarded by adequate guardrails meeting the requirements of 29 CFR 1915.71(j)(1) and (2).  On or about September 10, 2013, at the T.T. Barge Mile 237, LLC barge repair facility at 5790 N. River Rd., Port Allen, LA;    a)    Employees were exposed to a fall of approximately 16 feet from the deck of a Kirby barge to the deck of a floating dry dock below.  The fall hazard was created by approximately 25 feet of inadequate guardrail along the port side of the deck near the bow of the barge.  The top rail, a wire that was not taut, was approximately 28 to 29 inches above the deck of the Kirby barge.  And, the exposed employees were not observed to be wearing safety belts and life lines meeting the requirements of 29 CFR 1915.159 and 1915.160.    b)    Employees were exposed to falls of approximately eight feet because the primary ramp/catwalk used to the access the floating portion of the facility from the shore did not have adequate guardrails.
