When using a combination respirator (e.g., airline respirators with an air-purifying filter), employers must ensure that the assigned protection factor is appropriate to the mode of operation in which the respirator is being used. (a) In the Savage Range Systems facility located at 100 Springdale Road Westfield, MA: On or about November 27, 2013, in the employer's Lead Compliance and Respiratory Protection Programs, the Assigned Protection Factor for a 3M ADFLO PAPR welding helmet was listed as 1,000 without any evidence provided by the manufacturer - according to the manufacturer, an ADFLO PAPR welding helmet provides an APF of 25. The employer's Lead Compliance and Respiratory Protection Programs were not maintained in accordance with this subpart.

29 CFR 1910.134(h)(1): Respirators were not cleaned and disinfected using the procedures in Appendix B-2 of 29 CFR 1910.134 or equivalent procedures recommended by the respirator manufacturer: (a) In the Savage Range Systems facility located at 100 Springdale Road Westfield, MA: On or about November 27, 2013, a lead wipe sample of a 100 cm2 (square centimeter) area was taken inside the range technician's PAPR welding helmet with a measured concentration of 195.5 ug/100 cm2 (micrograms per square centimeter). This is in excess of the OSHA definition of a clean surface which is less than 25 ug/100 cm2. The employer did not ensure the employee's respirator was cleaned and disinfected in accordance with this subpart.
