The fall hazard to the employees was 24 feet above the ground.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder: a) Jobsite, 100 Chaucer Court, Moon Township, PA: On or about September 11, 2013, employees were using a 24 foot extension ladder to access a residential roof and did not maintain the 4:1 ratio. The pitch of the roof was 9 in 12 and the ladder was positioned even with the roof.

Santiago Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 315560193, citation number 1, item number 4 and was affirmed as a final order on August 3, 2011, with respect to a workplace located at 1800 Fifth Avenue, Pittsburgh, PA 15218.
