29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Jobsite, 100 Chaucer Court, Moon Township, PA:  On or about September 11, 2013, employees were working on a residential roof without fall protection.  The fall hazard to the employees was 24 feet above the ground.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder:  a) Jobsite, 100 Chaucer Court, Moon Township, PA:  On or about September 11, 2013, employees were using a 24 foot extension ladder to access a residential roof and did not maintain the 4:1 ratio.  The pitch of the roof was 9 in 12 and the ladder was positioned even with the roof.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a)  Jobsite, 100 Chaucer Court, Moon Township, PA:  On or about September 11, the employer did not provide a training program for his employees to recogize fall hazards and/or how to minimize these hazards.  Santiago Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 315560193, citation number 1, item number 4 and was affirmed as a final order on August 3, 2011, with respect to a workplace located at 1800 Fifth Avenue, Pittsburgh, PA  15218.
