State St. Bridge in Fremont, OH: The employer failed to ensure a safe means of access/egress was provided to employees working from a suspended scaffold system. During bridge restoration work, employees utilizing the suspended scaffold were climbing atop of the scaffolding rail, then using the bridge railing to pull themselves up and over onto the bridge deck. Employees accessing/egressing to and from the scaffold in this condition were exposed to falls of approximately 20 ft. to the river below.
