29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or another provision in paragraph (b) of this section which provides for an alternative fall protection measure:    Ortiz Roofing, at 650 W Park Avenue, Johnstown, CO: On 9/11/13, Ortiz Roofing did not ensure that each employee engaged in residential construction activities 6 feet or more above lower levels was protected by guardrail systems, safety net systems, personal fall arrest systems, or another provision in paragraph (b) of this section which provides for an alternative fall protection measure.  Employees of Ortiz Roofing were roofing a new construction single story home.  The employees were not protected from fall hazards through use of guardrails, safety net systems, or personal fall arrest systems.  This condition exposed employees to fall hazards of approximately eleven feet.    Ortiz Roofing, at 650 W Park Avenue, Johnstown, CO, was previously cited for violations of this Occupational Safety and Health Administration Standard, 29 CFR 1926.501(b)(13), which were contained in Item 1 of Citation 1 issued on April 24, 2013 (OSHA Inspection 901289) and in Item 1b of Citation 1 issued on October 16, 2009 (OSHA Inspection 313413080).

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder:    Ortiz Roofing, at 650 W Park Avenue, Johnstown, CO: On 9/11/13, Ortiz Roofing did not ensure that ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder.  Employees of Ortiz Roofing were roofing a new construction single story home.  Employees accessed the roof of the home using an extension ladder.  The ladder was positioned upon loose gravel.  The ladder was not secured to prevent displacement.  This condition exposed employees to a fall hazard of approximately ten feet ten inches.
