29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) Mezzanine, Harper Collins Publishing Warehouse, 1000 Keystone Industrial Park, Throop, PA: Employees removed sections of a mezzanine floor composed of corrugated metal sheets without any means of fall protection while working at a height of fourteen-foot, two-inches (14'2") above a concrete floor. Employees were exposed to a fall hazard, on or about September 24, 2013. In accordance with 29 CFR 1903.19(c), abatement certification is not required for this violation. Abatement was observed during the inspection.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l) as referenced by the note contained in 29 CFR 1926.602(d). (a) Employees operated powered industrial trucks (forklifts) daily and did not receive training and an evaluation of their ability to safely operate the equipment. Employees were exposed to struck-by and crushed-by hazards associated with forklift operations, on or about September 9, 2013. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.
