Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the employees were exposed to a struck by hazard while unloading granite stone bundles:   a) On or about 9/9/13, an employee was permitted to work in the fall shadow of two bundles of granite slabs. The granite bundles were inside a cargo box and while the employee removed the wood supports that were in between the two bundles they fell and trapped the employee.    Among other methods, one feasible and acceptable method to correct this hazard is to train the employees to stay out of the "fall shadow" while removing the wood supports.

29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposes an employee to injury and the defective or damaged item was not removed from service and an employee was using it before repairs and tests necessary to render the equipment safe were made:  a) On or about 9/9/13 - an employee was using two extension cords with a Sawzall to cut wood supports. The employee was exposed to electrical current, in that, one of the extension cords did not have a plug on the end of it. The female end of the plug was missing, therefore, the hot and neutral wires were connected to male end of the second extension cord.     b) On or about 9/9/13 - an employee was using two extension cords with a Sawzall to cut wood supports. The insulation on one of the extension cords was damaged which caused the three wires to protrude out of the insulation.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:   (a) on or about 9/9/13 - an employee was exposed to an electrical hazard, in that, an extension cord being used with a Sawzall had the ground prong missing.   (b) on or about 9/9/13 - an employee was exposed to an electrical hazard, in that, an extension cord, used with a Sawzall had a missing female end and the hot and neutral wires were connected to the male end of another extension cord, however; the ground wire was not connected.
