29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8m) or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b) of this section:    Employees performing roofing work on a roof 6 feet or more above ground are not protected from a fall hazard by a fall protection system.        The employer fails to assure effective protection against falls as required by the standard, as demonstrated on August 16, 2013, when three employees were working on a residential roof at 7421 Bella Foresta Place, Sanford, Florida 32771 without fall protection and were exposed to a fall hazard of 11 feet 1 inch.      Collis Roofing, Inc., previously committed a repeat violation of this standard on June 9, 2012, with respect to a workplace at 2120 White Jasmine Court, Apopka, FL, which was reclassified to a serious violation and became a final order on August 29, 2012 (Inspection No. 472520).      Collis Roofing, Inc., previously committed a second repeat violation of this standard on September 16, 2011, with respect to a workplace at 3820 Catania Place, Jacksonville, FL, which was reclassified to a single repeat violation and became a final order on December 28, 2011 (Inspection No. 315479931).       Collis Roofing, Inc., previously committed a repeat violation of this standard March 23, 2011, with respect to a workplace at 4257 Isle Vista Avenue, Belle Isle, FL, which was reclassified to a serious violation and became a final order on May 18, 2011 (Inspection No. 315498089).        Collis Roofing, Inc., previously committed a repeat violation of this standard on March 1, 2011, with respect to a workplace at 125 Montura Drive, Ponte Vedra Beach, FL, which was reclassified to a serious violation and became a final order on March 28, 2011 (Inspection No. 313882136).       Collis Roofing, Inc., previously committed a serious violation of this standard on August 1, 2008, with respect to a workplace at 13785 Herons Landing Way, Jacksonville, FL, which became a final order on October 8, 2008 (Inspection No. 311816813).      To abate this violation, the employer must ensure that its employees performing roofing work are protected from falls, and that no employee is allowed onto a roof 6 feet or more in height without adequate fall protection.  Pursuant to 29 C.F.R. 1903.19, the employer must submit documents describing the steps it is taking to ensure compliance, including a description of how these steps protect its employees from fall hazards.
