29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware shall be provided by the employer for isolating, securing or blocking of machines or equipment from energy sources:    (a)  Employees operating the CNC machines, such as but not limited to the Makino EDM Machine, Mori-Seki CNC Lathe, and the Haas CNC Mill, were not provided with locks and tags to attach to the energy isolating devices prior to cleaning inside the guards and changing the tooling.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees:     (a)  Employees operating the injection molding machines changed the molds, and cleaned out the "buttons" or "short shots" using a brass screwdriver, without first affixing a lockout device to each of the energy isolating devices.      (b)  Employees operating the injection molding machines, entered the robotic cells during setups without first affixing a lockout device to each of the energy isolating devices.      (c)  Employees operating the Mori-Seki CNC Lathe, the Makino EDM Machine, and the Haas CNC Mill, cleaned inside the guards with a rag and changed the tooling, without first affixing a lockout device to the energy isolating devices.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.147(c)(7)(i):  The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:    (a)  Employees operating the injection molding machines were not adequately trained on the energy control program, such as when and how to attach locks to the energy isolating devices, prior to changing the molds.      (b)  Employees operating the injection molding machines were not adequately trained on the energy control program, such as when and how to attach locks to the energy isolating devices, prior to entering the robotic cells.      (c)  Employees operating the Mori-Seki CNC Lathe and the Makino EDM Machine, and the Haas CNC Mill, were not adequately trained on the energy control program, such as when and how to attach locks to the energy isolating devices, prior to cleaning inside the guards and changing the tooling.         The employer was previously cited for a violation of this occupational safety and health standard or its equivalent standard which was contained in OSHA Inspection Number 108948, citation number 1, item number 2c, and was affirmed as a final order on  February 28, 2012, with respect to a workplace located at 852 Scholz Drive, Vandalia, Ohio.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):    (a)  Employees operating the injection molding machines changed the molds and cleaned the buttons or short shots inside the guards, without first isolating the energy sources.      (b)  Employees operating the injection molding machines entered the robotic cells without first isolating the energy sources. 29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.:    (a)  The point of operation guards on the Mori-Seki CNC Lathe and the Haas CNC Mill were not interlocked.        (b)  The chuck on the Emco Machine Lathe was not guarded.      (c)  The DoAll Vertical Band Saw had 1 ¼ inches of unguarded blade,       The employer was previously cited for a violation of this occupational safety and health standard or its equivalent standard which was contained in OSHA Inspection Number 108948, citation number 1, item number 6, and was affirmed as a final order on  February 28, 2012, with respect to a workplace located at 852 Scholz Drive, Vandalia, Ohio.
