29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:   A. Aaron Williams worksite located at 2717 Cheltenham, Toledo, Ohio: On or about September 11, 2013, the employer did not ensure employees wore proper eye protection while operating pneumatic nail guns to secure weatherproofing on the two story structure. Employees were exposed to potential flying object hazards while operating the pneumatic fasteners.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.  A. Aaron Williams worksite located at 2717 Cheltenham, Toledo, Ohio: On or about September 11, 2013, the employer did not ensure employees were protected from falls while installing weatherproofing on the east side of the two story residential structure.  The employees were exposed to a fall distance of approximately 18.00 feet to the lowest level.

29 CFR 1926.1053 (b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access.     A. Aaron Williams worksite located at 2717 Cheltenham, Toledo, Ohio: On or about September 11, 2013, the employer did not ensure the side rails of the extension ladder was extended at least three feet above the landing surface located on the second story of the two story residential structure.  Employees were exposed to falls while using the ladder to access the east side of the roof.
